Please fill out the following questionnaire to apply for the $500 High School Scholarship and/or the $500 First in the Family Scholarship. You may submit one application to be considered for both scholarships. But to be considered for the First in the Family (first-generation college bound) scholarship, you need to answer Yes to question 6 and complete question 7. This year's Application Deadline is Monday, April 22, 2013.
